Assessment of the new residency program for family physicians in Israel:knowledge, attitudes and self-efficacy in the care for diabetes mellitus, palliative care, geriatrics and psychiatry

רקע מדעי ומטרה:

The Israeli syllabus for the family physician residency was recently modified. The training program is aimed at adequately preparing the residents for their final year, in which their clinical and communication skills are integrated into everyday practice.

Our aim in this study is to evaluate the new syllabus and curriculum's influence on residents in the following clinical fields: diabetes mellitus (DM) as a representative of one the most common chronic diseases in the adult population in Israel, palliative care (PC), geriatric care (GC), and psychiatry (PSY).

שיטות:

We created a questionnaire to assess knowledge, attitudes and self-efficacy in four clinical fields: DM, PC, GC and PSY.
The knowledge aspect was assessed using MCQs from the national family medicine residency exams, whilst attitude and Self efficacy were assessed using validated standard questionnaires.
Two of Clalit’s family medicine departments participated in the study: Sharon-Shomron (Sosh) and Haifa-West Galilee (HWG).

תוצאות:

The first phase of our study included 140 residents:
The knowledge score for DM was significantly higher than all the other subjects
. The self-efficacy score for DM was significantly higher than all the other subjects. GC self-efficacy score was significantly higher than for PSY and PC.
.The attitude scores were significantly different as well

מסקנות:

Knowledge, self-efficacy and attitudes of family medicine residents is higher for DM than PC/GC/PSY, and there are further differences within each of the disciplines assessed.

Future studies will assess the baseline difference between residents and teachers, as well as longitudinal follow-up of residents training under the new syllabus.
